 Description of Forms
Rental Opportunities - contains a description of each of our rental areas and the hourly rental rate assigned to each. We list the rates for weekdays (Mon-Thurs) and the rates for the weekend (Fri-Sun) by MCRC Member and Non-Member status.
Party Packages - contains a description and cost of our party packages. The parties are booked in Community Rooms B or C. Due to space constraints, parties are limited to 30 people. This includes both adults and children. Requests for parties above 30 people may be accommodated if space is available. Additional guests will be charged $5 per person. An adult to child ratio of 1:8 (1:4 for Pool parties) must be maintained at all times throughout the duration of the party.
Rental Request Form - must be completed (including date and signature) and submitted by email, fax, mail or dropped off at the front desk during facility hours. Requests are typically reviewed within 2 business days and are subject to approval based based on facility and staff availability. Once a request is reviewed, you will receive a phone call. We firmly recommend that you not send out invitations until you receive a confirmation call from the Rental Coordinator. After your rental request is approved, a Rental Contract will be drawn up and arrangements will be made for you to sign the contract and pay for the rental fees within 48 hours.
Rental Handbook - contains information about the facility, the rental process and rental policies. Please note our contract change fee and cancellation policy.
Group Daily Pass Sign-In Sheet - complete this form if you have a group of non-members interested in purchasing daily passes (see ID Policy under the Information tab). If the participant is a youth under 18 years of age, the parent or legal guardian is required to sign next to the printed name. Please bring the sheet and payment with you to the MCRC front desk the day of your event. MCRC Members do not need to be included on this form. Members scan their ID cards to gain admittance.
Pool Rules - a copy of the Pool Rules must be signed and submitted with the Rental Request form if using the pools. Children 7 yrs of age and under must be directly supervised by an adult in the pools. We require 1 adult per 4 children for Pool Party Packages. Make sure that everyone that is in attendance is familiar with the pool rules before the day of your event.
Rental Evaluation Form - we encourage you to complete this form at the end of your rental. We are always interested in your thoughts about your time in our facility. All comments and suggestions are greatly appreciated!
